Purpose

Describe the systematic approach and when to perform a complete vs. focused assessment.

Sequence

General Survey

  • Appearance, level of distress, body habitus, hygiene

Neurological

  • LOC, orientation (person, place, time, event), GCS if indicated

Head & Face

Eyes, Ears, Nose, Throat (EENT)

Neck

  • JVD, tracheal deviation, lymph nodes, thyroid

Cardiovascular

  • Heart sounds (S1, S2, murmurs), peripheral pulses, capillary refill, edema

Respiratory

  • Breath sounds, respiratory effort, SpO₂

Abdomen

  • Inspection, auscultation (bowel sounds), percussion, palpation (tenderness, organomegaly)

Musculoskeletal

  • ROM, strength, gait

Integumentary

  • Skin color, temperature, turgor, lesions, pressure injury risk

Genitourinary / Elimination

  • Last BM, urine output, characteristics

Psychosocial

  • Mood, affect, support system, coping
