Purpose
Describe the systematic approach and when to perform a complete vs. focused assessment.
Sequence
General Survey
- Appearance, level of distress, body habitus, hygiene
Neurological
- LOC, orientation (person, place, time, event), GCS if indicated
Head & Face
Eyes, Ears, Nose, Throat (EENT)
Neck
- JVD, tracheal deviation, lymph nodes, thyroid
Cardiovascular
- Heart sounds (S1, S2, murmurs), peripheral pulses, capillary refill, edema
Respiratory
- Breath sounds, respiratory effort, SpO₂
Abdomen
- Inspection, auscultation (bowel sounds), percussion, palpation (tenderness, organomegaly)
Musculoskeletal
- ROM, strength, gait
Integumentary
- Skin color, temperature, turgor, lesions, pressure injury risk
Genitourinary / Elimination
- Last BM, urine output, characteristics
Psychosocial
- Mood, affect, support system, coping
