Overview

Implementation is the fourth phase of the nursing process. The nurse carries out the planned interventions and documents the care provided.

Types of Implementation Activities

  • Cognitive — Teaching, decision-making, problem-solving
  • Interpersonal — Therapeutic communication, coordination
  • Technical — Skills, procedures, treatments

  4. Right direction/communication
  5. Right supervision
  6. Right evaluation

Documentation Principles

  • Document after performing care (not before)
  • Be objective and accurate
  • Use approved abbreviations
  • Include time, signature, and credentials

Safety During Implementation

  • Verify patient identity (two identifiers)
  • Follow standard precautions
  • Confirm orders before carrying out
  • Reassess continuously
