Select the stool type that best matches the patient's observed or reported stool form. Use consistently for serial bowel documentation. Types 3 and 4 represent ideal consistency. Document the type in the patient's bowel record and initiate appropriate interventions.
Bristol Stool Form Scale (BSFS)
A seven-type visual classification of human faeces. Developed by Heaton and Lewis at the University of Bristol (1997). Types 3 and 4 are considered ideal; Types 1–2 indicate constipation; Types 5–7 indicate loose stools or diarrhoea. Used to objectively document stool form in clinical records and bowel management protocols.
