Assessment Tool: CRAFFT

The CRAFFT is a validated six-item screening tool for substance use disorders in adolescents aged 12–21. It screens for all substances — alcohol, cannabis, prescription drug misuse, and other drugs. The mnemonic covers six behavioural consequences of substance use: Car, Relax, Alone, Forget, Friends/Family, and Trouble. A score of ≥ 2 is a positive screen. Administer in a private setting without parents or guardians present, and explain confidentiality limits before beginning. Used in the D443 substance use assessment context (D443.10.2) for special history-taking considerations.

CRAFFT — Adolescent Substance Use Screen

A six-item validated screening tool for substance use disorders in adolescents (ages 12–21). Each "Yes" answer scores 1 point. A score of ≥ 2 is a positive screen indicating probable substance use disorder and the need for further assessment. The CRAFFT mnemonic covers the key behavioural consequences of substance use: Car, Relax, Alone, Forget, Friends/Family, Trouble.
