Have the patient select the face that shows how much they hurt right now. Read the instruction text aloud and point to each face when introducing the scale to the patient.
Faces Pain Scale – Revised (FPS-R)
A self-report pain intensity scale using six facial expressions rated 0–10 (even numbers). Validated for children aged 4–16 and adults who have difficulty with numeric scales. Developed by Hicks et al. (2001); freely available for non-commercial use from the IASP.
Administration: Point to each face and say: "This face shows no pain at all. The
faces show more and more pain. Point to the face that shows how much you hurt right now."

Reassess after pain management interventions. Trending scores over time supports evaluation of treatment effectiveness.
