Rate each of the seven domains based on the patient's current status. Implement the appropriate fall-prevention protocol based on the total score. Reassess with any change in condition, after a fall, or per institutional policy.

Johns Hopkins Fall Risk Assessment Tool (JHFRAT)

A seven-domain validated fall risk screen for adult inpatients. Developed at Johns Hopkins Hospital. Score 0–5 = Low risk; 6–13 = Moderate risk; ≥ 14 = High risk.

High-risk medications include: antiseizure drugs, benzodiazepines, diuretics, narcotics/opioids, sedatives/hypnotics, psychotropics/antipsychotics.
Patient care equipment includes: IV lines, catheters, chest tubes, surgical drains, supplemental oxygen devices, continuous monitoring leads.
