Mini-Mental State Examination (MMSE)

A 30-point validated cognitive screening instrument covering six domains. Originally developed by Folstein, Folstein, and McHugh (1975). The MMSE is a proprietary instrument owned by Psychological Assessment Resources (PAR). Score interpretation (general guidance)

24–30

No / minimal impairment

Orientation to Time (0–5)

Year (1), Season (1), Date (1), Day (1), Month (1)

Orientation to Place (0–5)

State (1), County/country (1), Town/city (1), Hospital/facility (1), Floor (1)

Registration (0–3)

Name 3 objects; ask patient to repeat them. Score 1 for each correct on first try.

Attention and Calculation (0–5)

Serial 7s (subtract 7 from 100 five times): score 1 per correct subtraction. Alternatively, spell "WORLD" backward (D-L-R-O-W).

Recall (0–3)

Ask patient to recall the 3 objects named in Registration. Score 1 per correct recall.

Language – Naming (0–2)

Name a pencil (1) and a watch (1).

Language – Repetition (0–1)

Repeat: "No ifs, ands, or buts." Score 1 for correct repetition.

Language – 3-Stage Command (0–3)

"Take this paper in your right hand (1), fold it in half (1), and put it on the floor (1)."

Language – Reading (0–1)

Show "CLOSE YOUR EYES" in large print. Score 1 if patient closes eyes.

Language – Writing (0–1)

Ask patient to write a spontaneous sentence. Score 1 if it contains a subject, verb, and makes sense.

Visuospatial – Copying (0–1)

Copy intersecting pentagons. Score 1 if all 10 angles are present and 2 must intersect.
