Answer all six items based on the patient's current status. If BMI cannot be calculated, calf circumference may be substituted for item F1 (≥ 31 cm = 3 pts; < 31 cm = 0 pts). A score of 11 or below warrants a full MNA assessment and dietitian referral.

Mini Nutritional Assessment – Short Form (MNA-SF)

A validated 6-item nutritional screening tool for community-dwelling and hospitalised older adults. Total score 0–14. Developed by Guigoz, Vellas, and Garry; adapted by Rubenstein et al. If BMI cannot be measured, calf circumference (< 31 cm = 0; ≥ 31 cm = 3) may be substituted for item F1.
