Assessment Tool: Nutritional Risk Screening 2002 (NRS-2002)

Complete the pre-screening first. If any pre-screening item is positive, proceed to the main screening. A total score ≥ 3 indicates the patient is at nutritional risk and requires a nutrition care plan. Add 1 point for patients aged 70 years or older.

Nutritional Risk Screening 2002 (NRS-2002)

A two-step validated nutritional risk screen for hospitalised patients. Developed by Kondrup et al. (2003); endorsed by the European Society for Clinical Nutrition and Metabolism (ESPEN). A total score ≥ 3 indicates nutritional risk requiring a nutrition care plan. Add 1 point if patient age ≥ 70 years.

Total score = Nutritional status score + Disease severity score + (1 if age ≥ 70)

Not at nutritional risk — re-screen weekly

Score ≥ 3

At nutritional risk — initiate care plan
