Assessment Tool: Numerical Rating Scale (NRS) — Pain

Ask the patient to rate their current pain on a scale from 0 (no pain) to 10 (worst pain imaginable). Reassess after any pain management intervention to evaluate effectiveness.

Numerical Rating Scale (NRS) — Pain

A validated, patient-reported pain intensity measure rated 0–10. Widely used in clinical settings for adults and children ≥ 9 years. Ask the patient to choose the number that best describes pain right now.

Administration: Ask the patient: "On a scale from 0 to 10, where 0 is no pain at all and 10 is the worst pain you can imagine, how would you rate your pain right now?"
