REEDA Scale

Five-domain wound healing assessment. Each domain scored 0–3; total range 0–15. Score 0 = optimal healing; higher scores indicate complications. Originally validated for perineal wounds and widely applied to surgical incisions.

Presence and extent of erythema measured from wound edges

E Edema

Swelling at or around the incision site

E Ecchymosis (Bruising)

Extent of bruising or discolouration around the wound

D Discharge

Type, color, amount, and character of drainage from the wound
