Assessment Tool: SCOFF Questionnaire

Read each question to the patient and record their yes or no response. A score of 2 or more positive answers suggests a probable eating disorder and warrants further clinical evaluation and referral to specialist services.

SCOFF Questionnaire

A 5-item yes/no screening tool for anorexia nervosa and bulimia nervosa. Developed by Morgan, Reid, and Lacey (1999) at St George's Hospital, London. Score ≥ 2 positive responses suggests a probable eating disorder and warrants further clinical evaluation.

Ask the patient to answer Yes or No to each of the following five questions:

S – Do you make yourself Sick because you feel uncomfortably full?
C – Do you worry you have lost Control over how much you eat?
O – Have you recently lost more than One stone (6 kg / 14 lb) in a 3-month period?
F – Do you believe yourself to be Fat when others say you are too thin?
F – Would you say that Food dominates your life?
