Stoma Type

Identify the type of ostomy to guide output expectations and appliance selection.

Stoma Construction

End stomas are more common post-permanent diversion; loop stomas are often temporary.

Stoma Color and Viability

Assess mucosal color under good lighting. Compare to normal pink-red baseline.

Stoma Height / Profile

Ideal stoma height is 1–2.5 cm above skin level (budded). Variation increases leakage risk.

Mucocutaneous Junction (MCJ) Integrity

Assess the suture line where stoma mucosa meets peristomal skin.

Peristomal Skin Assessment

Assess the skin within a 10 cm radius of the stoma base using SACS-aligned severity categories.

Pouching System Performance

Evaluate seal integrity and wear time to guide barrier selection and patient teaching.

Stoma Output

Assess output volume, consistency, color, and odor relative to expected norms for stoma type.

Patient / Caregiver Self-Care Ability

Evaluate independence with pouch emptying, pouch change, and problem-solving.
